AbstractDuring the last year, our study of transport airplane loss-of-control (LOC) mishaps has grown into a study of unmanned aerial vehicle (UAV) mishaps. Initially, the work was an application of the techniques to investigate transport accidents with sequences of precursors and events during the LOC mishap. We have been examining military UAV mishaps over the past few months and have examined sixty-nine mishaps.
Recently, we have expanded this study to include civil UAV mishaps and have examined 112 mishaps. Results to date indicate that loss-of-control, rather than midair collisions is the major issue with UAV operation. System-related issues include a lack of redundancy in flight control systems, electrical systems, and in propulsion systems. The primary crew-related problems appear to be a lackadaisical attitude among the crew as well as lack of sensory input from the vehicle to the control station.
